The BC Ministry of Advanced Education and Ministry of Health requires mandatory, daily monitoring for any individual who will access a workspace or field site under the current pandemic alert level. This is applicable for both research and teaching related activities. Please note that guidelines or content of this form may be revised at any time to reflect additional regulatory requirements at the Federal, Provincial, local or University level. This process is a critical component of compliance regulations allowing a return to work - failure to complete this sign-in accurately and consistently will violate Ministry of Health requirements and could result in the shutdown of all workspaces. Faculty/Instructors/Supervisors must maintain accurate records for review by the Dean’s Office/EHS/WorkSafe BC. |
